Scope of Services and Communication

Services may be provided via:

  • Face-to-face consultations

  • Telehealth (video) consultations

  • Phone consultations

  • Email communication (administrative and clinical support where appropriate)

Clients acknowledge that:

  • Email and phone communication may have limitations

  • Sensitive information is handled with care, however some risks exist with electronic communication

Informed Consent

Consent is obtained prior to all services and is ongoing.

Clients consent to:

  • Naturopathic assessment and treatment

  • Collection and use of personal and health information

  • Communication via phone, email, and telehealth

Consent is:

  • Written (via intake forms)

  • Verbally confirmed and documented in clinical notes

Nature of Naturopathic Care

Services are provided in a naturopathic capacity unless otherwise stated.

Clients understand that:

  • Naturopathy is complementary to medical care

  • It does not replace diagnosis or treatment by a medical practitioner

  • No guarantees of outcomes are provided

Collection of Personal Information

Information collected may include:

  • Name, contact details, date of birth

  • Medical history and current health concerns

  • Medications and supplements

  • Lifestyle, dietary and health-related information

  • Clinical notes and treatment plans

Information is collected via:

  • Intake forms

  • Consultations

  • Email or phone communication

  • Online booking systems

Use of Personal Information

Information is used for:

  • Providing healthcare services

  • Developing treatment plans

  • Communication regarding appointments and care

  • Legal and regulatory compliance

Information will not be sold or shared with third parties except:

  • With client consent

  • Where required by law

  • Where there is a serious risk to health or safety

Client records are stored securely using:

  • Encrypted, password-protected digital systems

  • Google Workspace and Cliniko (secure cloud-based platform)

  • Practice management software (where applicable)

Security measures include:

  • Password protection and access controls

  • Secure backups

  • Restricted access to authorised personnel only

Reasonable steps are taken to protect information from misuse, loss, unauthorised access or disclosure.

Retention of Records

Health records are retained in accordance with legal requirements:

  • Minimum of 7 years for adults

  • For minors, until age 25

Data Breach Management

In the event of a data breach, Jane Osborne and atLife Health Collective will:

  • Contain and assess the breach

  • Notify affected individuals if required

  • Report to the Office of the Australian Information Commissioner (OAIC) where applicable

Telehealth and Electronic Communication

Clients consent to communication via:

  • Video conferencing platforms

  • Phone

  • Email

Clients acknowledge:

  • Telehealth has limitations compared to in-person care

  • There are inherent risks with electronic communication

  • They are responsible for ensuring privacy on their end during consultations

Client Rights

Clients have the right to:

  • Access their personal information

  • Request corrections

  • Withdraw consent at any time

  • Make a complaint

All client information is treated as confidential and will not be disclosed without consent except:

  • Where required by law
